1. My appointment was cancelled after I missed the Covid questionnaire phone calls. I am on phone calls all day for work, and I had a good amount of phone calls I had to take after work as well, dealing with insurance companies. That's why I missed the calls. 2. I got my notification that my appointment was cancelled via email, yet I never got notification that I needed to submit anything to confirm the appointment via email. 3. When I called to ask about the cancellation, I was then asked the Covid questions over the phone. 4. When I got to the door, I was told I couldn't come in until the Covid email was answered; it was the exact same questions I'd just been asked over the phone. Furthermore, why was this not sent to me via email in the first place, if this was what was required? When I finally got in, the front desk girl proceeded to ask me the questions about my medical history loudly and in the presence of other customers. While other customers were in the lobby, she read out a list of over a dozen medical conditions asking me to state whether I had them or not. Finally, I just suggested I fill the form out myself, but I was already surprised at the lack of concern for personal privacy.
